The "Paperwork" Behind YouTube Monetization

In case you ever wondered what Google asks for in the event of unclear licensing and rights in a video, I got my first "elevated" claim request today. The first level of inquiry is a basic text form in YouTube itself, where you can input additional information for reviewers to assess.
